Site audit
Walk the site with your phone. Say what you see, photograph it, move on. The report writes itself as you go and leaves as a PDF before you leave the gate.

In the office
Afterwards, the walk is a report: every item with its place, its photograph and its words, as a PDF to share with the site manager and to file, and a fix list with owners and dates. Look at it before it leaves. The moment it does — a PDF, a read-only link or a fix link — the walk is issued and its words are frozen; until then you may correct a place or a line, or strike an item out. After that nothing is deleted: an item raised in error is voided with a reason and keeps its number.
On site
On the phone: name the place, take the photograph, say or type what you found — dictated in any language, the transcript lands in the box for you to read and correct. Say what it is: good practice, an observation, an action with an owner and a date, or stop. The screen and the fix list read in English, Polish, Romanian, Lithuanian or Bulgarian; the report itself is always in English, one document for everyone who receives it.
What it never does
It never decides what you saw. The finding, its severity and what should be done about it are the auditor's words, not KASQ's.
What it counts
What one run counts against a plan's monthly questions is on the tool's page and in the tools overview, both written from the same table as the plans page.
